Mom did not die from an acetaminophen overdose. She died of another, unrelated problem elsewhere in her body that was not of her own doing. Before the litigious crowd chimes in I'll state for the record that while the hospital missed it because the symptoms were masked by the painkillers, she almost surely would have died regardless of treatment. It helps explain why she was so desperate for painkillers, though.

It is a small, but tangible, comfort to know that Mom did not cause her own death.
